Scheduling an Office Visit or
Procedure
Our Patient Scheduling Specialists can schedule office visits and procedures.
Please call (309) 672-4980 and a Patient Specialist will help you schedule your
next appointment.
If you know your insurance requires a referral
or physician order from your primary care physician - please contact your
primary care provider directly and request it from them. They can fax us the
referral directly to (309) 671-2931. To ensure you well coordinated care
between your primary care provider and our providers - we require a referral or
physician order from your primary care provider to schedule all procedures.
What you should bring to your
Next Appointment
Please bring a list of all medications you are currently taking. List the exact
name, dosage and number of times a day you take each medication. You can also
bring in a bag containing all of the medications you are taking and be prepared
to provide information regarding the dosage and number of times you are taking
each medication. You will also want to bring in your most recent insurance
card(s) as well.
If you have had tests performed that were
requested by other physicians that you think will be helpful in your treatment -
please call our office at least 5 business days before your next appointment and
let us know the test performed and the location. we can then request a copy of
the results and have them here for your visit with our providers. You can also
bring in a copy of those test results if you have them.
Medication Refills
If you received your prescription from one of our providers, please
call your pharmacy
to request a renewal of your prescription. This is the most efficient
way to get your prescription refilled. Your pharmacy will contact us to renew
your prescription.
Please remember to request all medication
refills at least 3 business days
before your prescription runs out. Because we need to need to review
your chart before renewing your prescription, we need adequate time to receive
approval for mediation renewals. Please do not wait until your prescription
runs out as it may cause a break in your medication therapy.
We may request you to come in and see your
provider. If your prescription can not be renewed without first seeing your
provider, we will contact you directly to let you know and schedule your
appointment.

Getting Test Results
We will notify you of your results by mail, by telephone, or at your next
scheduled appointment. Results may not be available to use for up to 14
business days after the test is performed - please do not call our office for
results before that time as we will not have your result to give to you. You
should receive results in the mail within 1 week after we receive them.

X-RAYS:
Our office will make every effort to contact you as soon as we receive and
review your x-ray or Nuclear Medicine test results. However, it can take up to
14 business days for us to receive your results. If you have not received a
call within 2 business days from your test, please

672-4980 to reach a medical assistant to inquire about
your results.
Lab
Results:
Our office will make every effort to
contact you as soon as we receive and review your laboratory or stool test
results. However, it can take up to 14 business days for us to receive your
results. If you have not received a call or your results in the mail within 20
business days, please contact our office at (309)

to reach a medical assistant to inquire about your results.
Procedure Results:
Gastroenterology provides patients with their Endoscopy results and
discharge directions when they leave the facility after their procedure. If a
biopsy was taken, your pathology/tissue report will be mailed to your within 14
business days of your procedure. We will also send a copy of your results to
your primary care provider or your referring physician if you provided us with
that information. Please call our office if you have not received your results
within 20 business days.
