Patient Overview

At Illinois Gastroenterology Institute, you are the focus of everything we do. From easy-access, free covered parking to promptly scheduled appointments, we are committed to providing you with an accurate diagnosis and high-quality treatment for your individual needs.

IGI serves adult patients throughout downstate Illinois, with many of our physicians sub-specializing in various aspects of gastroenterology. You can find us at the bottom of Main Street hill in downtown Peoria in the Illinois Medical Center building. Our office is on the 5th floor, located adjacent to Central Illinois Endoscopy Center, where many of your diagnostic procedures can be performed.

To provide you with an efficient check-in process, we ask our patients to bring their completed medical history and other signed forms with them. You will receive a copy in the mail or you can access them below. Please note that you will not fill them in online. Because many of the forms require a signature, they can be printed, completed, signed and brought with you to your appointment.
