Office Visit Appointments
Our patient scheduling specialists schedule office visits and procedures. If your insurance requires a referral or physician order from your primary care physician for office visits, please contact your primary care provider directly and request it from them. They can fax us the referral directly to (309) 671-2931. If your insurance does not require this for office visits, please call (309) 672-4980.
To ensure well-coordinated care between your primary care provider and our providers, we require a referral or physician order from your primary care provider to schedule all procedures.
What you should bring to your appointment
Please bring a list of all medications you are currently taking. List the exact name, dosage and number of times a day you take each medication. You can also bring in a bag containing all of the medications you are taking and be prepared to provide information regarding the dosage and frequency. Please bring your most recent insurance card(s) as well.
AS OF MAY 1ST 2014, ALL CO-PAYS WILL BE COLLECTED PRIOR TO YOUR APPOINTMENT. If you are not able to pay your co-pay prior to your appointment, please call our office to be rescheduled.
If you have had tests performed by other physicians that you think will be helpful in your treatment, please call our office at least five business days before your next appointment and let us know the test performed and the location. We can then request a copy of the results and have them here for your visit with our providers. You can also bring in a copy of those test results if you have them.
